We recommend downloading and completing both pages of our liability waiver before your appointment. Bring your completed copy with you to help make check-in easier.

Printed waivers are also available at the rage room if you need to complete one when you arrive.

If a minor will be dropped off, please review the waiver with their parent or guardian ahead of the visit. Call us before arrival with any questions about completing the form or drop-off arrangements.
